Since all the moonsight-drama, can there ever be an 100% accurate conversion between the two?

The computer model will give you an idea but in reality the start/finish of the month is based on sighting so yes it can never be accurate.
You can further enhance your chances by using:
a) Islamic date
b) Islamic Year
c) Day of the week
e.g. if you convert my DOB it gives the wrong result but when you also include Tuesday it gives exact results.
