Saying Bismillah before al-fatihah in all rakats is a confirmed sunnah, so you should do this.

However, they're are two positions regarding saying the bsimillah before a surah. So do what you think is right for that.

Here is the Hanafi position (for seekersguidance)
In summary, the ruling of reciting the basmala in the Hanafi school is as follows:

    1. It is a confirmed sunna to recite the basmala (saying Bismillah…) before the Fatiha quietly in every rakat of the prayer, for other than the follower (who does not recite). [Haskafi, Durr al-Mukhtar]

    2. It is not a confirmed sunna to recite the basmala between the Fatiha and a Sura. This is not disliked, however. [ibid]

    3. If one recites the basmala between the Fatiha and Sura, it is good (hasan). [Ibn `Abidin, Radd al-Muhtar] This is only, however, if one is reciting from the beginning of the surah. [Ibid.]

Therefore, given that the basmala is not necessary (wajib) but a confirmed sunna, one does not perform forgetfulness prostrations for leaving it out, because these are only required if one leaves a necessary (wajib) act. [Shurunbulali, Maraqi al-Falah; Tahtawi, Hashiya `ala Maraqi al-Falah]

    And Allah knows best.

    Wassalam,

Bismillah before Fatiah is a confirmed sunnah, so you should do it.
Bsimillah before surah is very optional. Do it if you want, or dont do it, wont effect validity of your salah.

My tajweed sheikh (Hafs / 3asim) told me that Bismillahir Rahmaanir Raheem in sura Fatiha is the first ayah & must be read aloud. Though other Hafs / 3asim teachers do not read it aloud. ALLAH says in Quran that Fatihah is seven verses (15:86). The reasoning for reciting aloud is that Sura Fatiha in salat must be to a certain standard & an error may well cancel your salat (recitation of sura Fatihah). "So why risk it by merely whispering the bismillah", was the advice.
Death asked Life; "Why does everyone hate me but love you?" Life replied; "Because I am a beautiful lie & you are a painful truth".
So Life asked Death; "Then, why do the believers prefer you over me?" Death replied ,"Because you are a painful deception and I am a beautiful relief."
